Has my loved one's memory performance changed?

Alzheimer's disease, as well as other forms of dementia, are characterized, among other things, by a memory performance that deteriorates over time. The timely detection of changes is essential for early diagnosis.

What can you do if a loved one can't remember things as well as they did two years ago? If the person no longer knows what day or month it is? The scientifically validated online questionnaire is the first important step toward a timely diagnosis of dementia and contributes to early detection. Use the questionnaire to assess the affected person's memory performance if you have known them for at least two years. If you have checked boxes for the seven questions, you will receive a clear overall assessment. If a certain threshold is reached, we will provide a recommendation for further steps.

Scientific Evidence:

Unlike many other questionnaires, this one uses a proven scientific method. The basis is the so-called IQCODE Questionnaire (The Informant Questionnaire on Cognitive Decline in the Elderly – translated: „Questionnaire for Informants on Cognitive Decline in the Elderly“). The IQCODE is a validated and globally used questionnaire that allows for the estimation of memory decline compared to a previous level of functioning with the help of an informant.Jorm et al. 1989, Jorm et al. 2004).

The IQCODE questionnaire is recommended in dementia diagnostic guidelines.The National Institute for Health and Care Excellence – NICE).

The informant questionnaire on our website is based on the German short form of the IQCODE with 7 questions, which is from Ehrensperger et al. validated in a 2009 study.

Privacy Notice:

The survey results will be stored completely anonymously for the purpose of evaluating the digital offering, without any traceability back to you. After the evaluation, this data will be deleted. If you wish to use the reminder function afterward, your email address will be stored encrypted in a database and cannot be linked to the results.
