Preparing for the survey
This document lists, in chronological order, all the key tasks involved in the survey process. You can check off completed tasks on the right side of the table.
Further information about the project is included in the participant information sheet, which you can send to potential study participants. This sheet is available in two versions: one for family caregivers and one for people with memory impairments. The latter is written in slightly simpler language, but the content of the two versions is the same.

To verify the inclusion criteria, a screening using the MMSE—and, if necessary, supplemented by the MoCA—must be conducted. The screening may only be conducted if you have obtained the informed consent of the person with cognitive impairments.
If a person with cognitive impairment is eligible to participate based on the inclusion criteria and the screening results, you must inform and educate both the person and their family caregiver about digiDEM Bayern, obtain consent to participate in the study, and then conduct the interview.

Testing using the MMSE and MoCA (Screening)
As a research partner, you will need the document containing instructions for administering the Mini-Mental State Examination (MMSE), the corresponding scoring sheet, and the print templates.
If the Mini-Mental State Examination (MMSE) yields a score of 24 or higher, a second screening must be conducted using the Montreal Cognitive Assessment (MoCA). As a research partner, you will again need the document containing instructions for administering the MoCA, the corresponding scoring sheet, and the print templates. We recommend folding the MoCA scoring sheet in the middle, below the animal pictures, so that the participant cannot see the subsequent tasks in advance.

The forms for claiming the project lump sum are provided below. Claim forms are available for general organizations, small businesses, and individuals. Please use the form that applies to your organization or to you. To fill out the claim form electronically, you will need Adobe Acrobat Reader DC, which you can download for free from the Internet, or a comparable program. Please submit invoices using only the provided form to rechnungen@digidem-bayern.de.

The form for claiming the Screeningtagpauschale is provided below. Claim forms are available for general organizations, small businesses, and individuals. Please use the form that applies to your organization or to you.
For each MoCA and MMSE screening conducted as part of a dementia screening day, you will receive 8 euros as a research partner—regardless of whether the screening results in study participation. (If a screening is negative, there is no need to enter the results in REDCap or fill out the master data sheet.) Submission of the participant list for the screening day is required for reimbursement. The list of participants is attached to the reimbursement form document.
